NC State Controlled Substance License Application (NC DHHS Form 225): This is the state of North Carolina application form you submit to obtain a controlled substance license. There is no on-line form for this license. This is the FIRST step in the process of obtaining a RESEARCHER license. Obtaining the state license number is required prior to requesting a federal license.

Federal Controlled Substance Application (DEA Form 225): This is the on-line federal application form you submit to obtain a RESEARCHER license. This is a different license from a practitioner license for physician or veterinary clinical care use of controlled substances (which uses DEA form 224). A practitioner license is for clinical care' a researcher license is for experimental use.
